Blue Crab Access Reactivation ~ July 2016

Dear Home Health Providers:

Delmarva Foundation, the District’s Long Term Care Supports contractor, is pleased to inform you that beginning June 30, 2016, home health agencies are able to access the Delmarva Blue Crab database to retrieve assessment documents for beneficiaries in its care. In order to access information for a beneficiary, the agency will need to identify one staff person responsible for retrieving information from Blue Crab, and complete the attached access form. Return the access form, along with a census of the beneficiaries for whom your agency is currently providing Personal Care Aide (PCA) services to Roopa Chakkappan (Roopa.Chakkappan@dc.gov).

Thereafter, Delmarva will contact you with a password, and dates for you to attend a training on Blue Crab’s enhanced features. If you have submitted the requested information and have not heard back from Delmarva, please feel free to reach out to Roopa Chakkappan (Roopa.Chakkappan@dc.gov).
